Healthcare Coordinator
Reporting directly to the Operations Manager, the Healthcare Coordinators oversee all aspects of case management and care provided to the company's clients. Including assessing the needs of all new and current clients and to plan and manage the care necessary for each individual and ensure that field staff provide quality, safe and exceptional care and service to the clients. The role includes liaison with medical practitioners and health officials in relation to clients needs, and to create a firm working business relationship with these persons.
Care Coordinator
Reporting directly to the Operations Manager, the role of the Care Coordinator is to coordinate, liaise and roster field staff to ensure that the needs of all clients are met through ongoing monitoring of the service provided. This is an integral team position as the coordinator is the primary contact for the client, Healthcare Coordinator and the care worker and is responsible for ensuring the delivery of high quality services on an ongoing basis.
